Understanding Gum Disease

Gingivitis vs Periodontitis — what is the difference?

Gum disease progresses in stages. Gingivitis is reversible — the gums are inflamed but bone is not yet damaged. Periodontitis is more serious — the infection has spread below the gumline, destroying the bone and ligaments that support the teeth. Without treatment, periodontitis leads to tooth mobility and eventually tooth loss.

Stage 1 — Reversible

Gingivitis

Gums are red, swollen and bleed on brushing. Caused by plaque accumulation at the gumline. Treated with professional scaling and polishing and improved home care. Fully reversible at this stage.

Bleeding gums Red swollen gums Bad breath
Stage 2 — Requires Treatment

Periodontitis

Infection has spread below the gumline — destroying the bone and connective tissue supporting the teeth. Pockets form between teeth and gums. Requires deep cleaning (scaling and root planing) to stop progression.

The Procedure

What happens during deep cleaning?

Deep cleaning (also called scaling and root planing or SRP) is a two-part procedure performed under local anaesthesia. It removes tartar and bacterial toxins from below the gumline and smooths the root surfaces to prevent bacteria from reattaching.

1

Gum Assessment

Pocket depth measurements are taken around every tooth using a periodontal probe. X-rays assess bone levels. The extent of the disease is mapped and the treatment plan is explained.

2

Local Anaesthesia

Local anaesthesia is administered to ensure the procedure is comfortable — especially important for deeper pockets.

3

Scaling Below the Gumline

Ultrasonic and hand instruments remove calculus deposits from root surfaces below the gumline — areas that regular scaling cannot reach.

4

Root Planing

Root surfaces are smoothed (planed) to remove bacterial toxins embedded in the root surface and to create a clean surface that the gum can reattach to.

5

Review Appointment

4–6 weeks after deep cleaning, pocket depths are re-measured to assess healing and gum reattachment. Further treatment is planned if pockets remain deep.

FAQs

Gum disease and deep cleaning questions.

Is deep cleaning painful?+
Deep cleaning is performed under local anaesthesia — the procedure itself should not be painful. Some soreness and sensitivity for 3–5 days after is normal and managed with over-the-counter pain relief.
How is deep cleaning different from regular scaling?+
Regular scaling removes plaque and tartar from above the gumline. Deep cleaning goes below the gumline into the gum pockets — targeting the infection that causes periodontitis.
Can gum disease be cured?+
Gingivitis (early stage) is fully reversible. Periodontitis (advanced stage) cannot be cured — but it can be controlled. Deep cleaning stops the progression and maintains the bone that remains.
How many sessions does deep cleaning take?+
Most patients need 2–4 sessions — typically one per quadrant (quarter of the mouth) to allow thorough treatment and patient comfort. Sessions are often spaced a week apart.
